After creating the page, you have to install and config the Google Map plugin accordingly to the plugin documentation:
http://tech.reumer.net/google-maps/documentation-of-plugin-googlemap/

The plugin should be published in the module position 56.

Module Position 50, 51, 52, 53 and 54

Issue: When I add a Custom HTML module to position-50, the Custom HTML module in position-51 disappeared. However the Custom HTML module for position 52-54 remained.

In the Template Configuration Parameters -> Footer Configuration Parameters -> Footer Row 2 please change the 'Extend Column 1 to column 2' to 'not extend'.
